Venous Doppler Ultrasound of the Legs

Also called: Lower-Limb Venous Duplex • DVT Scan • Venous Reflux Study

Venous Doppler combines ultrasound images with blood-flow assessment. It can look for a deep-vein thrombosis (DVT), investigate leg swelling or map faulty valves and reflux in varicose veins.

A DVT scan and a varicose-vein reflux study are not interchangeable. One looks urgently for clot, while the other is a detailed functional map—often performed standing—to guide planned venous treatment.

Do Not Wait for a Routine Scan if Symptoms Are Urgent

Seek emergency care immediately for:

  • Sudden or unexplained breathlessness, chest pain, fainting or collapse.
  • Coughing blood, a very fast heartbeat or severe breathing difficulty.
  • New chest symptoms when you have leg swelling, a known DVT or recent major surgery or immobility.

Seek same-day urgent clinical assessment for:

  • New swelling of one leg, especially with pain, tenderness, warmth or colour change.
  • Leg symptoms after surgery, prolonged immobility or travel, active cancer, pregnancy or recent childbirth.
  • A painful, hard, red superficial vein—particularly if redness is spreading towards the groin or behind the knee.
  • A bleeding varicose vein. Lie down, elevate the leg and apply firm direct pressure; get urgent help if bleeding is heavy, does not stop or causes weakness or faintness.

A leg ultrasound cannot diagnose or exclude a pulmonary embolism in the lungs. Emergency chest symptoms require immediate clinical assessment and may need ECG, blood tests, CT pulmonary angiography or another appropriate test.

What is Venous Duplex or Doppler Ultrasound?

A small probe sends high-frequency sound waves through gel on the skin. Grey-scale ultrasound shows the vein walls and any visible material within the lumen. Colour and spectral Doppler show the direction and pattern of moving blood. No radiation, needle, contrast dye or injection is required.

The radiologist also applies gentle, controlled probe pressure. A normal vein usually flattens completely under compression; a vein containing thrombus may not. Compression, appearance and blood-flow findings are interpreted together rather than relying only on colour on the screen.

“Doppler” Can Mean Two Different Leg-Vein Examinations

A request should state whether the question is acute DVT, chronic swelling or venous reflux. A negative limited DVT scan does not provide the detailed valve map needed before varicose-vein treatment, and a planned reflux study should not delay urgent assessment of possible acute DVT.

Which Venous Scan Do I Need?

DVT or compression ultrasound Looks for thrombus in the deep veins when there is acute swelling, pain or clinical concern. It is usually performed lying down and follows a defined proximal or whole-leg protocol.
Venous reflux or insufficiency study Tests valve function and maps reverse flow in deep, superficial and connecting veins. It is often performed standing or with the leg dependent so gravity can reveal reflux.
Superficial venous thrombosis assessment Examines a tender, hard superficial vein and determines clot extent and its distance from the deep venous junctions. Deep veins are also checked when clinically indicated.
Post-thrombotic or chronic swelling study Looks for residual obstruction, recanalisation, valve damage and chronic changes after a previous DVT. Findings are correlated with other causes of swelling.
Vein mapping for another procedure Measures the course and calibre of suitable superficial veins before selected bypass, access or venous procedures. This is a distinct mapping protocol.

The exact protocol, one or both legs and the veins examined depend on the referral, symptoms and local clinical pathway. Bring the original request rather than asking only for a generic “leg Doppler.”

Why Might the Scan Be Requested?

New one-sided swelling To assess for DVT when one leg becomes swollen, painful, warm or tender.
Varicose veins To identify faulty valves, reflux pathways and veins relevant to treatment planning.
Long-standing swelling To look for chronic venous obstruction, previous thrombosis or venous insufficiency.
Skin change or ulcer To assess venous reflux contributing to pigmentation, eczema, hardening or a leg ulcer.

Other indications can include:

  • A raised D-dimer with clinical concern about DVT.
  • Follow-up of a previous DVT when repeat imaging will change management.
  • Symptoms after a long journey, hospital admission, plaster cast or reduced mobility.
  • Pregnancy or the post-partum period with symptoms suggesting venous thrombosis.
  • Suspected superficial thrombophlebitis or thrombosis near a deep-vein junction.
  • Recurrent varicose veins after previous surgery, ablation or sclerotherapy.
  • Planning for endovenous treatment, surgery or selected vascular access procedures.
  • Checking a treated vein or procedure-related complication when clinically required.

Leg swelling is not always caused by a clot or faulty valves. Infection, injury, lymphoedema, medicines, joint disease and heart, kidney or liver conditions may need separate evaluation.

Which Veins and Features Are Examined?

Deep veins The common femoral, femoral and popliteal veins are assessed in standard proximal protocols. Calf veins are added in whole-leg or targeted examinations according to the referral and pathway.
Superficial veins The great and small saphenous veins, visible tributaries and a symptomatic superficial vein are examined when varicose veins or superficial thrombosis are relevant.
Venous junctions The saphenofemoral junction in the groin and saphenopopliteal region behind the knee are checked for reflux, anatomy and proximity of superficial thrombus to the deep system.
Perforator veins These connect superficial and deep veins. Enlarged or incompetent perforators may be mapped when they contribute to varicosities, skin change or an ulcer.
Compressibility and lumen Each accessible vein is compressed at intervals and examined for visible thrombus, wall change, residual channel or recanalisation.
Blood-flow pattern Colour and spectral Doppler assess spontaneous flow, response to breathing and temporary flow changes produced by gentle calf compression or other manoeuvres.
Reflux and valve function Reverse flow is provoked and timed in named vein segments. A complete map identifies its source, route and relationship to visible varicose veins.
Nearby soft tissues A fluid collection, Baker's cyst or another visible abnormality may occasionally explain symptoms, but venous ultrasound is not a complete muscle, joint or soft-tissue examination.

How Should I Prepare?

Lower-limb venous Doppler usually requires no fasting and no full bladder. Eat, drink and take regular medicines normally unless your treating clinician gives different instructions.

Clothing Wear loose garments that can be removed or raised from the groin to the ankle. A gown or drape may be provided while only the area being examined is exposed.
Compression stockings Ask whether to remove them before arrival. They must be removed for scanning, but the timing can matter for a reflux study and should follow local instructions.
Anticoagulants and other medicines Do not stop any prescribed anticoagulant, antiplatelet or other medicine for an external ultrasound unless the treating clinician specifically instructs you.
Standing and mobility Tell the team if you become faint, cannot stand safely, have severe pain, need mobility support or are at risk of falling. The position and manoeuvres can be adapted.
  • Bring the referral and previous Doppler, CT, venography or treatment reports.
  • Bring an up-to-date medicine list and mention previous DVT, PE or vein treatment.
  • Tell the team about pregnancy, recent surgery, cancer, immobility, long travel or a family clotting history.
  • Do not apply creams or heavy moisturiser immediately before the scan if this can be avoided.
  • Tell the team about open ulcers, dressings, infection, fragile skin or severe tenderness.

What Happens During a DVT Scan?

1 The clinical question is confirmed

The radiologist reviews the affected side, symptom timing, clinical probability, D-dimer when available, previous clots, anticoagulation and relevant risk factors.

2 You lie on the examination couch

The leg is exposed from the groin downward while dignity is protected. You may lie on your back, bend the knee, turn partly onto your side or sit up for calf and behind-knee views.

3 The veins are compressed

Gel is applied and the probe presses at short intervals along accessible veins. Pressure may be uncomfortable over a tender or swollen area but is brief and carefully controlled.

4 Blood flow is tested

Colour and Doppler traces evaluate flow. The radiologist may ask you to breathe normally, hold briefly or tolerate a gentle squeeze lower down the leg to assess flow response.

5 The required extent is completed

Depending on the protocol, assessment may stop after the proximal veins or continue through calf veins and a specific superficial abnormality. One or both legs may be scanned.

6 Findings are reported promptly

Images and measurements are saved. Confirmed acute thrombosis or another urgent finding is communicated through the appropriate clinical pathway so management is not unnecessarily delayed.

A straightforward one-leg DVT scan often takes about 15–30 minutes. Bilateral, whole-leg or technically difficult examinations can take longer.

What Happens During a Varicose-Vein Reflux Study?

A reflux study is more extensive than a DVT check. The aim is to reproduce the effect of gravity, test valve closure in named vein segments and create a map that explains the visible varicose veins.

1 Visible veins and symptoms are reviewed

Previous treatments, aching, swelling, skin change, bleeding or ulcer location help focus the map. The relevant leg is inspected before or during scanning.

2 You are positioned with the leg dependent

You may stand on a stable step or use a tilted or seated position. The team supports safe balance and adapts the examination if standing is not possible.

3 Deep and superficial veins are checked

Patency and previous thrombotic changes are assessed before valve function is interpreted. Great and small saphenous veins and relevant junctions are traced.

4 Reflux is provoked and timed

Calf compression and release, a brief bearing-down manoeuvre or machine-assisted compression may be used. Reverse flow is observed and measured rather than inferred from vein size alone.

5 The reflux pathway is mapped

Incompetent junctions, saphenous segments, tributaries and relevant perforators are documented, including vein course and calibre when needed for treatment planning.

6 The map informs—not dictates—treatment

The vascular clinician combines the duplex result with symptoms, skin findings, goals, prior treatment and medical suitability before recommending observation or an intervention.

A detailed bilateral reflux study may take 30–60 minutes or longer. The duration depends on the number of abnormal pathways, recurrent disease, ulcers, mobility and previous procedures.

What Do Common Report Terms Mean?

Compressible / no DVT demonstrated The examined vein segments flatten normally and show no sonographic evidence of DVT. This conclusion applies to the protocol and veins assessed, not automatically to every pelvic or calf vein.
Non-compressible vein / intraluminal thrombus A vein does not collapse fully and contains material consistent with clot. Location, extent, occlusion and imaging features are described to guide clinical management.
Acute or recent DVT The clot has features suggesting a recent event. Ultrasound appearance can support timing but cannot assign an exact date in every case; symptoms and previous imaging remain important.
Chronic post-thrombotic change Wall thickening, scarring, a smaller irregular vein, residual material, collateral veins or valve damage suggests a previous thrombosis rather than a new occlusive clot.
Recanalisation / residual thrombus Blood-flow channels have formed through or around older thrombotic material. Residual ultrasound abnormality does not by itself mean that anticoagulation has failed or must continue.
Superficial venous thrombosis Clot is present in a superficial vein. The report states its length and proximity to deep-vein junctions because these influence risk assessment and treatment.
Venous reflux / incompetence Reverse flow persists beyond the expected valve-closing response in a tested vein segment. The source and distribution matter more than simply seeing a dilated vein.
GSV / SSV Abbreviations for the great saphenous vein and small saphenous vein—the main superficial trunks commonly mapped in varicose-vein assessment.
SFJ / SPJ The saphenofemoral and saphenopopliteal junction regions, where superficial veins join the deep system in the groin and behind the knee.
Incompetent perforator A connecting vein allows abnormal outward or reverse flow between deep and superficial systems. Its location and relationship to skin change or an ulcer affect significance.

What Can Venous Doppler Not Determine Reliably?

  • Whether a clot has travelled to the lungs; suspected pulmonary embolism needs separate assessment.
  • Every small calf-vein clot when only a proximal protocol has been performed.
  • Pelvic or abdominal vein detail when depth, bowel gas or body habitus limits ultrasound access.
  • The exact age of thrombus in every case, especially without earlier imaging for comparison.
  • All causes of leg pain or swelling, including lymphatic, joint, muscle, nerve and systemic disease.
  • The correct duration of anticoagulation from ultrasound appearance alone.
  • Which varicose-vein treatment is best without clinical assessment and a complete reflux map.
  • Every vein segment when severe oedema, ulcers, dressings, tenderness, limited mobility or calcification restricts access.

A negative initial scan does not always end the investigation. Depending on clinical probability, D-dimer, the protocol used and persistent symptoms, the treating team may arrange repeat ultrasound or different imaging. Do not stop anticoagulation or ignore worsening symptoms without clinical advice.

What Happens After the Scan?

The gel is wiped away and you can usually return to normal food, fluids, medicines and activity. If an acute clot is found, the radiologist communicates the result through the agreed urgent pathway so the treating team can assess bleeding risk and begin appropriate management.

A negative result is interpreted with the Wells clinical probability, D-dimer and the scan protocol. Selected patients need repeat proximal ultrasound after several days; others need evaluation for an alternative cause. The clinician—not the scan alone—makes that decision.

For varicose veins, the report provides a reflux map for the vascular clinician. Not every visible vein needs treatment, and treatment choice depends on symptoms, complications, anatomy and personal suitability.

Myth vs Fact

Myth DVT always causes obvious pain and swelling.
Fact Symptoms vary and can be mild or absent. Clinical probability and appropriate testing—not symptoms alone—determine whether DVT is likely.
Myth Doppler requires an injection of coloured dye.
Fact The colour is generated by the ultrasound machine from moving blood. Routine venous Doppler uses no injection, contrast dye or radiation.
Myth Visible varicose veins mean there is a deep-vein clot.
Fact Varicose veins usually reflect superficial valve failure and reflux. DVT is a different condition, although both can coexist and need the correct scan protocol.
Myth A negative leg scan rules out every clot and pulmonary embolism.
Fact A result applies to the examined veins and protocol. Pulmonary embolism requires separate assessment, and selected patients need repeat or alternative imaging.

Why must I stand for a varicose-vein scan?

Gravity fills the leg veins and makes abnormal reverse flow easier to reproduce and map. If you cannot stand safely, tell the team; a dependent seated or tilted position may be used.

Can I have venous Doppler during pregnancy?

Yes. Diagnostic ultrasound uses no ionising radiation. New one-sided leg swelling or chest symptoms during pregnancy or after childbirth require prompt medical assessment rather than routine booking.

Can a negative scan still require a repeat?

Yes. A repeat scan may be advised when clinical probability remains significant, D-dimer is positive or only proximal veins were examined. Follow the treating team's plan and return sooner if symptoms worsen.

Does a reflux study decide which treatment I need?

It supplies the anatomical and functional map. A vascular clinician combines it with symptoms, skin findings, previous treatment, medical history and your preferences before recommending care.

When will I receive the result?

The radiologist prepares a report for the referring clinician. Acute DVT and other urgent findings are communicated promptly through the appropriate pathway. Ask how the final result will be discussed.
