Plantar fasciitis is a common load-related cause of pain at the inner underside of the heel, where the plantar fascia helps support the arch. Pain is often worst with the first steps after sleep or sitting, then may ease before returning with prolonged standing or activity.

Despite the traditional “-itis” name, persistent symptoms are not simply ongoing inflammation. Diagnosis is usually clinical, and most people improve without surgery through load management, suitable footwear, stretching, progressive strengthening and time.

Not every painful heel is plantar fasciitis

  • Severe pain followed a fall, crush or road collision, the foot looks deformed, or you cannot bear weight.
  • A sudden pop caused bruising, swelling, loss of push-off or inability to stand on tiptoe.
  • The heel is hot, red and swollen with fever, spreading redness, a wound, discharge or feeling very unwell.
  • The foot is cold, pale, blue, numb or weak, or pain rapidly escalates in a tense swollen foot.
  • You have diabetes, neuropathy or poor circulation and develop a blister, ulcer, puncture, warmth, swelling or new deformity.
  • Pain is progressive at rest or night, associated with unexplained swelling, weight loss, several inflamed joints or significant nerve symptoms.

These findings may indicate fracture, Achilles or plantar-fascia rupture, infection, nerve or circulation problems, inflammatory disease or another diagnosis. Do not assume first-step pain makes them harmless.

What Is the Plantar Fascia?

The plantar fascia is a strong band of connective tissue running from the heel bone towards the toes. It supports the arch and tightens as the toes extend, helping the foot become a stable lever for push-off.

Repeated load is normal and necessary for tissue health. Symptoms can develop when current demand exceeds capacity—for example after a sudden increase in walking, running, jumping or standing, a change in shoes or surface, or reduced recovery. The tissue can become painful without being torn.

Why Might It Develop?

Load increase

More steps, standing, mileage, hills, speed, jumping or a return after inactivity.

Recovery mismatch

Training or occupational load accumulates faster than sleep, rest and tissue adaptation.

Calf and ankle capacity

Limited ankle movement or insufficient calf and foot strength may alter plantar load.

Footwear or surface

Abrupt changes, worn cushioning, thin soles or prolonged hard-floor exposure may provoke symptoms.

Body load

Higher body weight or a recent weight change can increase load with each step.

Occupation and sport

Standing work, running and repeated jumping can create high cumulative demand.

Flat or high arches can occur with or without plantar pain. Foot shape alone does not establish the cause, and the condition is not a personal failure or proof that the foot is “badly aligned.”

  • Footwear: cushioning and comfort can reduce symptoms; there is no single best shoe for every foot.
  • Taping: rigid or elastic foot taping may provide short-term relief and can help test whether support changes symptoms.
  • Heel cups: may improve comfort but do not treat every plantar or heel-pad cause.
  • Prefabricated or custom orthoses: can supplement stretching and strengthening; they should not be used as the only treatment.
  • Night splints: may help persistent morning first-step pain by keeping the ankle and fascia from resting fully shortened.

An expensive custom device is not automatically superior. Fit, comfort, adherence, shoe compatibility and measurable response matter.

A small, short-lived symptom response may be acceptable in some programmes, but a clear limp, escalating pain or a marked next-morning deterioration means the plan needs adjustment.

Recovery and Persistent Symptoms

Plantar fasciitis often improves slowly. Meaningful recovery may take several months, especially when symptoms have been present for a long time or work and sport loads cannot easily be modified.

  • Track function—first-step pain, walking duration, work tolerance and exercise capacity—rather than expecting pain to disappear overnight.
  • Reassess adherence, total daily load, exercise dose, footwear and the original diagnosis if progress stalls.
  • Persistent symptoms do not automatically mean surgery; they may indicate that load, diagnosis or treatment selection needs refinement.

Is plantar fasciitis the same as plantar heel pain?

Plantar heel pain is a broader symptom description. Plantar fasciitis or fasciopathy is one common diagnosis when the location, timing and examination fit.

Why is it worse with my first steps?

The fascia and calf rest in a shortened, unloaded position, then receive a sudden load when standing. This pattern is common but not diagnostic by itself.

Is a heel spur causing the pain?

Not necessarily. Spurs occur in many painless feet and are better viewed as an attachment-site adaptation than a measure of pain.

Should I stop walking completely?

Usually not. Reduce the dose that creates a sustained flare while maintaining tolerable daily movement, unless another diagnosis requires protection.

Can I run with plantar fasciitis?

Should I walk barefoot?

Are strengthening exercises safe?

Will a night splint help?
