Common term: High Blood Pressure

Blood pressure is the force of blood against the artery walls. Hypertension means that this pressure is persistently higher than the healthy range—not simply high during one stressful moment.

Most people feel completely well. Correct technique, repeated readings and the setting in which they were taken are therefore essential before a diagnosis or treatment decision is made.

Seek Emergency Care Immediately If

  • A very high reading accompanies chest pain, severe breathlessness, collapse or a new severe back pain.
  • There is new weakness or numbness on one side, facial droop, difficulty speaking, confusion or loss of balance.
  • There is sudden loss or major change of vision, a seizure, reduced consciousness or an unusually severe headache.
  • A repeated reading remains above 180/120 mmHg and any new concerning symptom is present.
  • During pregnancy or soon after birth there is severe headache, visual disturbance, pain below the ribs, vomiting, sudden swelling, breathlessness or a reading of 160/110 mmHg or higher.

If a reading is unexpectedly above 180/120 mmHg, sit quietly and repeat it after at least one minute. With any warning symptom, arrange emergency care immediately rather than repeatedly checking the machine. A persistently very high reading without symptoms still needs prompt same-day clinical advice.

What Do the Two Numbers Mean?

Systolic pressure

The first or upper number. It measures pressure when the heart contracts and pushes blood into the arteries.

Diastolic pressure

The second or lower number. It measures pressure while the heart relaxes between beats.

The unit is millimetres of mercury, written as mmHg. Either number can be important. The pulse shown by the device is a separate measurement and does not replace clinical assessment.

Why One Reading Is Not Enough

Blood pressure changes from minute to minute. Recent activity, pain, anxiety, poor sleep, a full bladder, talking, caffeine, tobacco, temperature and incorrect cuff technique can all alter a reading. Diagnosis is usually based on a pattern rather than the highest number seen once.

Hypertension is usually silent

Headache, dizziness and nosebleed are not reliable screening symptoms. Many people with substantially raised pressure feel normal, while these symptoms often occur for unrelated reasons. Measurement is the only way to know.

How to Measure Blood Pressure Correctly

  • Use a clinically validated upper-arm device with a cuff that fits the arm correctly.
  • Avoid exercise, tobacco, caffeine and alcohol for at least 30 minutes beforehand.
  • Empty the bladder, then sit quietly for at least five minutes.
  • Sit in a supportive chair with the back supported, feet flat, legs uncrossed and body relaxed.
  • Place the cuff on bare skin, not over clothing, with the supported arm at heart level.
  • Do not talk, use the phone or move during the measurement.
  • Take two readings at least one minute apart and record both unless the clinician advised another protocol.

Bring the device to a consultation so its cuff, technique and readings can be compared with the clinic equipment. Wrist and finger devices are more position-sensitive and are not the usual first choice when a suitable upper-arm cuff can be used.

Clinic, Home and 24-Hour Readings

Clinic measurement

Allows a trained assessment, repeat readings, arm comparison and examination, but anxiety or recent activity can raise the result.

Home monitoring

Shows the pattern during ordinary life when a validated device, correct cuff and consistent technique are used.

Ambulatory monitoring

A portable device records pressure repeatedly over about 24 hours, including during sleep, and can confirm clinic–home differences.

A Common Home-Monitoring Schedule

When home monitoring is used to confirm a diagnosis, a common protocol is two consecutive seated readings at least one minute apart, morning and evening, for at least four days and ideally seven. The first day is excluded and the remaining readings are averaged. Follow the exact schedule supplied by your clinician.

Do not check continuously in response to anxiety or selectively record only the lowest numbers. Note the date, time, arm, readings, pulse and any unusual symptom or circumstance.

How Is Hypertension Diagnosed?

A widely used adult pathway suspects hypertension when repeated clinic pressure is 140/90 mmHg or higher and confirms it when the average daytime ambulatory or home pressure is 135/85 mmHg or higher. WHO similarly uses repeated readings on different days at or above 140/90 mmHg for clinical diagnosis.

Why online charts may disagree

Guidelines use different labels below 140/90 mmHg. Some call 130–139/80–89 stage 1 hypertension, while the 2024 European guideline places much of this range in an “elevated blood pressure” category. The underlying principle is consistent: cardiovascular risk rises gradually, and interpretation depends on repeat readings and the person’s overall risk—not a label alone.

White-Coat and Masked Hypertension

White-coat effect

Clinic readings are higher than the home or ambulatory average. It still deserves follow-up because risk factors and the pattern can change over time.

Masked hypertension

Clinic readings appear acceptable but home or ambulatory pressure is raised. It can be missed if only clinic measurements are considered.

Out-of-office monitoring helps distinguish these patterns from measurement error and sustained hypertension.

What Is Postural Hypotension?

Some people experience a meaningful pressure drop on standing, causing dizziness, weakness, blurred vision or falls. Sitting and standing measurements may be needed, particularly in older adults, people with diabetes, those with falls or anyone with postural symptoms. A low standing pressure can change the safest treatment target even when seated readings are high.

Why Does High Blood Pressure Matter?

Persistently raised pressure can gradually injure arteries and the organs they supply. The risk depends on how high the pressure is, how long it has been present and the person’s other risk factors.

  • Stroke, transient neurological events and vascular cognitive impairment.
  • Heart attack, heart failure, rhythm problems and thickening of the heart muscle.
  • Chronic kidney disease and leakage of albumin into urine.
  • Damage to the retina and vision.
  • Narrowing, weakening or enlargement of arteries in different parts of the body.

Primary and Secondary Hypertension

Primary hypertension

The most common form. It develops through a combination of age, genetics, body composition, diet, activity, sleep, environment and vascular changes rather than one identifiable disease.

Secondary hypertension

Raised pressure caused or driven by another condition, such as selected kidney, hormonal, sleep or blood-vessel disorders, or by a prescribed or non-prescribed substance.

A secondary cause is considered more strongly with sudden onset, young age, very high or difficult-to-control pressure, an abrupt deterioration, unusual laboratory findings or a suggestive history. Not everyone needs every hormonal scan or specialist test.

Tests look for existing organ effects, cardiovascular risk and clues to a secondary cause. A normal result does not mean raised pressure can be ignored; it means complications have not been detected by that particular test.

Everyday Measures That Support Blood-Pressure Care

  • Reduce excess salt and highly salted packaged or preserved foods.
  • Build meals around vegetables, fruit, pulses, whole foods and an appropriate overall energy intake.
  • Choose sustainable physical activity after considering heart, balance, joint and other medical limitations.
  • Work toward a healthy weight and waist trajectory without crash diets or unsupervised fasting.
  • Avoid tobacco exposure and discuss alcohol use honestly.
  • Protect sleep and seek assessment for loud snoring, witnessed pauses or marked daytime sleepiness.
  • Use stress-management strategies that are realistic and repeatable.

These measures complement clinical care. They do not replace prescribed treatment when it is needed, and no prescribed plan should be stopped or changed because a few readings improve.

Treatment and Targets Are Individual

A clinician considers the confirmed pressure, age, pregnancy, kidney function, diabetes, previous stroke or heart disease, postural symptoms, frailty and overall cardiovascular risk. Some people need structured lifestyle follow-up alone initially; others also need prescribed treatment from the outset.

Clinic, home and ambulatory targets are not identical. A lower number is not automatically safer if it causes dizziness, falls, reduced kidney perfusion or other problems. Use the written target and measurement method agreed for you.

Hypertension During Pregnancy and After Birth

High blood pressure may predate pregnancy, develop after 20 weeks or occur with pre-eclampsia. Pregnancy uses different monitoring, laboratory and treatment pathways. It can also become serious after delivery, so new warning symptoms in the postnatal period should not be dismissed.

Anyone planning pregnancy should review the diagnosis and current prescriptions beforehand. Do not stop or change prescribed treatment without the obstetric or physician team’s advice.

When Is Specialist or Higher-Centre Referral Appropriate?

  • Severe pressure or symptoms suggest acute heart, brain, kidney, eye or aortic injury.
  • Pregnancy, suspected pre-eclampsia or severe postnatal hypertension is present.
  • Pressure begins unusually young, rises suddenly or remains difficult to control despite a verified plan.
  • A secondary kidney, hormonal, sleep or blood-vessel cause is suspected.
  • There is significant kidney disease, heart failure, previous stroke or complex postural symptoms.
  • Required monitoring or multidisciplinary services are beyond local capability.

Should I use a wrist blood-pressure monitor?

Does headache prove my pressure is high?
