Frozen shoulder does not usually begin as an instantly deformed or acutely swollen joint. Do not attempt forceful manipulation
after an injury or try to push a suspected dislocation back into place. Emergency cardiac and stroke symptoms should not be
attributed to a known shoulder condition.
What Becomes “Frozen”?
Ball-and-socket joint
The humeral head moves within the shallow glenoid socket to position the arm.
Joint capsule
A flexible sleeve surrounds the joint and normally allows a large range of movement.
Capsular inflammation
Early irritation contributes to pain, including pain at rest and at night.
Capsular contracture
The capsule thickens and tightens, mechanically limiting movement in several directions.
Rotator interval
This front-upper capsular region is commonly involved and contributes to loss of external rotation.
Rotator cuff
The tendons may be normal, coexist with age-related change or have a separate injury; they are not the capsule itself.
The Central Clinical Clue: Active and Passive Stiffness
Active movement
How far you can move the arm using your own muscles. Pain, weakness, fear and stiffness can all reduce active range.
Passive movement
How far the shoulder moves when the arm is supported and gently moved by the examiner. Frozen shoulder restricts this
range as well as active movement.
External rotation
Turning the forearm outward while the elbow remains near the side is characteristically restricted, often early and
disproportionately. No single measurement confirms the diagnosis by itself.
Behind-the-back reach
Fastening clothing, reaching a back pocket or personal hygiene may become difficult as internal rotation and elevation
reduce. This movement can be painful and should not be forced during examination.
Shoulder-blade compensation
The shoulder blade may move earlier to help lift the arm, making the apparent range look better than the ball-and-socket
movement actually is.
A Painful Rotator Cuff and a Frozen Shoulder Need Different Exercise Doses
Rotator-cuff pain may limit active lifting while passive joint movement remains relatively available. Frozen shoulder
restricts both. Aggressive strengthening of a highly irritable frozen shoulder may worsen pain, while treating every weak
painful shoulder as frozen can delay recognition of a tendon tear or nerve problem.
How Frozen Shoulder Commonly Evolves
Pain-predominant or “freezing” phase
Pain gradually intensifies, night pain is common and movement begins to reduce. The shoulder may be very irritable, so
forceful stretching can provoke prolonged flare-ups.
Stiffness-predominant or “frozen” phase
Pain may become less dominant while restriction remains substantial. Dressing, grooming, reaching and sleep can still
be difficult, and progressive mobility work often becomes more tolerable.
Recovery or “thawing” phase
Movement gradually returns and activity can be expanded. Strength and confidence may lag behind range, and some people
retain a degree of stiffness.
Why the stages are not a calendar
Phases overlap, symptoms fluctuate and diabetes or a secondary cause may prolong recovery. Treatment should follow the
current pain, irritability, function and trend—not a date calculated from first onset.
Recovery is usually measured in months rather than days and may extend beyond a year. Statements that every shoulder resolves
completely by a fixed deadline are not reliable; improvement should be monitored through pain, sleep, function and measured range.
Why Does Frozen Shoulder Develop?
Primary or idiopathic
No single trigger is identified. This is common and does not mean that the pain lacks a physical basis.
Diabetes
Frozen shoulder is more frequent in people with diabetes and may be more prolonged or resistant. Steroid injections
can temporarily raise glucose, so monitoring and an individual plan are important.
Thyroid and other associations
Both underactive and overactive thyroid disease are associated with frozen shoulder. Parkinson's disease and some
cardiovascular conditions are also reported associations, but a shoulder diagnosis does not prove any of them.
After injury or surgery
Pain, protection and prolonged immobilisation after a fracture, operation or other shoulder problem can lead to secondary
stiffness. The original injury and healing restrictions must be respected during rehabilitation.
Neurological illness or immobility
Reduced arm use after stroke or another neurological condition may contribute to stiffness, but pain and weakness in this
setting require a broader assessment rather than automatic labelling as frozen shoulder.
Diabetes and thyroid reviews should follow the person's medical history and routine care needs. Frozen shoulder by itself is not
an instruction to order every blood test or thyroid ultrasound.
What Else Can Cause a Painful Stiff Shoulder?
Glenohumeral osteoarthritis
Ball-and-socket arthritis can restrict active and passive movement. X-rays usually show joint-space loss, osteophytes
or other characteristic bone changes.
Rotator-cuff tear or pain
Pain and weakness may severely limit active movement, while passive range is often better preserved unless secondary
stiffness has developed.
Calcific tendinopathy
Calcium within a rotator-cuff tendon can cause an acutely painful shoulder and protective stiffness. X-ray or ultrasound
may identify the deposit.
Fracture, dislocation or post-traumatic stiffness
A trauma history changes the imaging and rehabilitation pathway. Movement restrictions may be necessary while bone or
soft tissue heals.
Inflammatory or infected joint
Rheumatoid arthritis, crystal disease and infection can cause pain and reduced movement. Fever, warmth, systemic illness
or several inflamed joints requires prompt medical evaluation.
Neck or nerve condition
Cervical radiculopathy and other nerve problems can cause shoulder-region pain, tingling or weakness but do not usually
create the same capsular passive restriction pattern.
What Happens During an Orthopaedic Assessment?
- History: gradual or traumatic onset, night pain, loss of function, diabetes, thyroid disease, previous surgery, fracture, injection or prolonged sling use.
- Active range: elevation, outward rotation, inward rotation and behind-the-back reach are assessed within tolerance.
- Passive range: the examiner gently evaluates ball-and-socket movement while accounting for shoulder-blade compensation.
- Strength: rotator cuff and arm function are tested as pain permits; apparent weakness may improve when pain is controlled.
- Neck and nerves: arm sensation, reflexes or neck movement may be checked when symptoms include tingling, numbness or pain below the elbow.
- Comparison: the other shoulder provides context, although bilateral frozen shoulder can occur.
The diagnosis is primarily clinical. A single painful movement or scan feature is not enough; the pattern of global passive
restriction and exclusion of important alternatives are central.
Which Tests or Scans Might Be Needed?
X-ray
Commonly used when imaging is appropriate to exclude ball-and-socket arthritis, an unrecognised fracture, calcific
tendinopathy or another bone problem. A normal or near-normal X-ray can support the clinical diagnosis.
Ultrasound
Not required to prove frozen shoulder. It may assess rotator-cuff tendons, the bursa, biceps tendon or calcific deposits
when another condition could coexist, and can guide a joint injection or selected hydrodilatation.
MRI
May show capsular or rotator-interval changes and can assess the rotator cuff, cartilage, bone marrow and deeper structures.
It is usually chosen when the diagnosis is uncertain or another finding would change treatment—not routinely for every case.
Blood tests
Selected according to history when diabetes, thyroid disease, inflammation or infection needs evaluation. A normal blood
test does not confirm or exclude frozen shoulder by itself.
Ultrasound Can Find a Rotator-Cuff Abnormality Without Explaining the Stiffness
Tendon changes become common with age and may coexist with adhesive capsulitis. If both active and passive movement are globally
restricted, a minor tendon finding should not automatically replace the capsular diagnosis. Imaging answers a question; it does
not substitute for movement examination.
What Can I Do Safely?
- Continue gentle daily use rather than keeping the arm completely still, unless a fracture, operation or other injury has specific restrictions.
- Move to a tolerable stretch, not repeated sharp pain. The acceptable dose differs between a highly painful phase and a later stiffness-predominant phase.
- Use the unaffected arm to assist comfortable elevation or rotation only after the technique has been checked.
- Break household and work tasks into shorter periods and bring frequently used objects within an easier reach temporarily.
- Support the arm with a pillow when sleeping if comfortable; prolonged sling use without a defined reason usually increases stiffness.
- Heat may help movement feel easier, while a wrapped cold pack may help after an irritable session. Protect the skin.
- Discuss pain medicines with a clinician or pharmacist if you have kidney, liver, stomach, heart or bleeding problems, are pregnant or take anticoagulants.
- If you have diabetes, maintain regular glucose care and ask how illness, reduced activity or a planned steroid injection may affect monitoring.
Treatment Is Matched to Pain, Stiffness and Function
Education and pacing
Understanding the slow but usually improving course helps set realistic goals. Treatment aims to protect sleep and function
while restoring movement without repeatedly provoking long flare-ups.
Physiotherapy
Gentle mobility and pain management are emphasised when irritability is high. Stretching range, shoulder-blade control and
strength can progress as pain settles and motion becomes more tolerant.
Pain medicines
Short-term options are considered after checking medical history, current medicines and individual risk. Medicine may make
sleep and rehabilitation easier but does not mechanically release the capsule.
Intra-articular steroid injection
A carefully placed joint injection may reduce pain, particularly earlier in a painful course, and help participation in
exercise. Benefit is not guaranteed; infection, anticoagulants and temporary glucose elevation require consideration.
Hydrodilatation
Image-guided fluid is introduced into the shoulder joint to distend the tight capsule, usually with local anaesthetic and
sometimes steroid. Results vary, and rehabilitation after the procedure is important.
Higher-centre procedural review
Persistent major pain and disability despite appropriate non-operative treatment may lead to discussion of manipulation
under anaesthesia or arthroscopic capsular release. Benefits and risks require specialist counselling.
